TPS62095R design related issues

I am using TPS62095RGTT as the power converter, where 5V input from a usb supply is converted to 3.5v. I think i have designed the layout according to the datasheet. But after soldering the board I am having some problems.

1. In one of the board, the output keeps following the input from 2v to 5.5v.

2. In the 2nd board, the voltage remains at 2.5v

One thing may be that since the IC package is vqfn, I may be soldering it incorrectly. So before I throw away the board and design a new one I wanted to make sure that I did not any issues with the pcb layout itself. I have attached the schematic and the layout. I hope somebody can give me some insight.


Also I have a question as to why the FB(5) pin remains at .8V. It says that internally it compares with a .8v reference. Even when my output is at 5v or at 2v, the feedback point is always at .8v.
